3/24/2023 EnchantaVaughn Approved Behavior: Client says he feels good and is having a good day despite everything that is happening in the community. Client was thankful that his counselor provides one on one help when he needs it and expresses happy thoughts about being in session. Client enjoys receiving good advice about what he should do to avoid lashing out or having an episode by utilizing deep breathing skills and different kind of coping strategies. Client openly discussed his feelings about his current state and how he feels about his mental disorder as well as his continued aggression. Behavior: Client appeared to be in a good mood as evidence by him participating in family activity without conflict. At times within the session, client was displaying aggressive behaviors such as yelling at his mother when discussing ways to communciate with his mother. Intervention: QMHP reflected with client on his day, reviewing both good and bad experiences. QMHP modeled proper positive behavioral expectations for the client to practice and implement in the home. QMHP supported client displaying positive social interaction with authority. QMHP modeled proper positive behavioral expectations for the client to encourage additional layers of support. Intervention: QMHP implemented individualized behavior management (i.e. being cooperative, asking for permission before leaving the room, and review conflict resolution steps). QMHP assisted client with reflective thinking. QMHP utilized active listening as information was conveyed. QMHP communicated with client on displaying appropriate control when he is faced with challenges in the home. QMHP commended client on his efforts to maintain his ISP objectives throughout the day. Response: Client was attentive and open with counselor about his feelings and how to identify them. Client shared that he believes he will be able to master his feelings soon and control his anxiety with more practice. Client demonstrated expectations and responded well to feedback. Client verbalizes positive feelings about himself but recognizes areas he needs to work on. Client was observed in his seat and waiting for direction. Client interacted well within his home environment. Response: Client reported a decreased stress level due to his modulation of social contact, and the success of this was reviewed. Client implemented the positive reinforcement provided by his counselor and is able to stay motivated and enjoy his day. Client exhibited good manners after earning incentive for his hard work. Client reduces sadness and finds more excitement in learning about his family history and mental health. Client continues to make progress with ISP objectives.
